My Progress Plus supports people in retaining their independence in their own home, within their own community. We have developed a comprehensive system of assessment, therapeutic response, environmental adaptation, monitoring and evaluation.

The purpose of this service is to provide a holistic programme of client centred care in peoples own homes. We aim to support individual life styles, activities and life skills of people, by addressing their physical, mental health, fitness, mobility and individual life challenges thus empowering them to recover and/or maintain their independence with choice.

As part of this service we are able to provide convalescence, respite care and rehabilitation opportunities in our care homes as a “stepping stone” to people returning to their own homes following hospital stay. In essence we can be a key catalyst to support the “improved patient care pathway” within our communities.

Restorative Therapeutic Intervention at Home

We want you to meet your full potential for health and wellbeing whatever your circumstances. By helping you to compliment your life with a range of therapy options you could be on the way to an improved quality of life.

We understand the importance of the relationship between body and mind, and will strive to help you achieve harmonious balance. Our professional team are committed to giving you all the help you require, so that the improvements you make are restorative, therapeutic and lasting. We appreciate that remaining in your own home will also help you to improve your quality of life.

Physiotherapy

Our dedicated team of Physiotherapists are here to help with low level rehabilitation. These types of programmes are designed for people suffering from immobility, painful joints, Parkinson’s Disease, Motor Neurone Disease, Strokes, Dementia or other neurological dysfunction.

Through comprehensive assessment we will identify and provide specific rehabilitation programmes for people following elective hip and knee surgery, post trauma ie falls, all adapted and tailor made to suit your individual needs and within your own familiar living environment, making recovery easier and gaining independence quicker. For those individuals who require additional support following discharge or indeed to prevent hospital admission we can provide a therapeutic residential service.

Occupational Therapy

Our service is committed to supporting your independence, and therefore through a comprehensive Occupational Therapy assessment any physical, cognitive and/or psychological needs can be identified and the appropriate solutions and action plans put in place. This may include environmental adaptations within your own home enabling you to remain independent for longer.

Therapeutic Care Assistants

Our therapeutic care assistants are trained to work alongside our physiotherapists and occupational therapists by actively delivering and assisting exercise and activity plans specifically designed for your individual programme of care, whether it be for rehabilitation, enhancing daily living activities or regaining skills.

Maintenance and Building Team

the Occupational Therapy service has direct referral links to our building and maintenance team and there fore you can be assured that any recommendations made or action plans drawn up are of the highest standard.

Community Nurse Practitioners

As part of our multi-disciplinary team we have Community Nurse Practitioners (RMN’s) providing support and guidance. This ensures an enhanced holistic approach to promoting your independence and choice.

Complementary Therapies

Our team of Complementary Therapists offer Therapeutic Massage, Aromatherapy, Healing and Reflexology. You can receive a tailor made programme of therapeutic treatment options to assist your recovery or simply promote relaxation. The benefits of complementary therapies are vast, but generally alleviates sleep disturbance, back neck and joint pain, headaches, anxiety, combat against stress related illness, skin and hair problems, low energy levels, recovery from surgery or any type of trauma. In addition, many people experience a feeling of wellbeing, self confidence and inner harmony. Complementary therapies can also support personal growth and development.
