Intermittent Assessment and Treatment Services
Cornwall Care provide specialist Intermittent Assessment and Treatment (IAT) services from The Green in Redruth. The service provides six treatment beds and works alongside community health and social care teams and community and district hospitals. The key objectives of the multiagency teams providing the service are to:

• Identify clients at risk and provide alternatives to hospital admission through a rapid response
• Provides solution focused therapeutic interventions geared to re-enablement and restoration of the client’s abilities and confidence to continue to live independently at home
• Identify clients at risk and deliver assertive, accelerated and safe transfer of clients from hospital to home
• Demonstrate a reduction in acute hospital admission rates for this client population
• Demonstrate a delay in clients with physical frailty or dementia transferring to long term residential or nursing care
• Provide information, training and support to relatives and carers of clients of the service.
• To work collaboratively with health and social care partners to ensure flexibility in each individual's care pathway.

The IAT service is managed by an experienced Home Manager and a team of trained support workers providing focused rehabilitation, restorative treatment and care on a 24 hour basis. The clients IATcare plan will be shared and agreed by PCT and ASCS professional staff and case managed by a Cornwall Care Network Lead Nurse Practitioner. Where appropriate, Cornwall Care’s My Progress Plus, physiotherapy and occupational therapy service will provide assessment and treatment including a home discharge follow up visit.

Supported living
We specialise in providing bespoke supported living services for people with learning disabilities, mental health needs, autism, physical disabilities and people with neurological conditions. We have a range of accommodation options across the county including self contained flats which are available to clients through a license agreement or assured shorthold tenancy. We provide a fluid support network which is adjusted with agreement with the client and their health and social support agencies to ensure an optimum balance of independence and support. Our personalised approach means that we can offer care and support up to 24 hours a day in the client's preferred setting.
